Medical Cannabis Renewal Patient Form

Patients that have been authorized through The Panday Group can renew their medical cannabis authorizations through this direct portal.

  • Please provide the mailing address we can send your original personal production authorization.
  • Our health practitioner will call you at the phone number provided
  • Please indicate your choice.
  • All patients are able to purchase medical cannabis from a licensed producer, even if they have a grow license. LPs will allow growers to purchase from them till Health Canada approves their license. Once a request has been submitted please register directly with the LP of choice.
